[ixpmanager] Duplicate ports after drop-in replacement of a switch

Hello,
We replaced a switch with one that was malfunctioning and after that we have duplicate ports in IXPM.
Note that between the replacement we have turned the switch to inactive.
What is the proper way to do a replacement in our switches with regards to IXPM.
P.S.
Each switch port has a record from live snmp and an extra record in the db.
