[ixpmanager] Duplicate entries on IXP Manager world map / user list

Hi,

Could someone please remove two stale entries from the IXP Manager
community world map and user list?

Both are legacy names for exchanges that now appear correctly under CANIX.

To remove:

1. "Quebec City Internet Exchange" (shortname: QIX Quebec)
   - Duplicate of "CANIX Quebec city" (CANIX YQB)
   - Both point to the same PeeringDB record:
https://www.peeringdb.com/ix/4604
   - Its IX-F link is also wrong - it points to ixp/266, which is Montreal,
not Quebec City

2. "Ottawa Gatineau Internet Exchange" (shortname: OGIX)
   - Duplicate of "CANIX Ottawa" (CANIX YOW)
   - Both point to the same IX-F record:
https://ixpdb.euro-ix.net/en/explore/ixp/811/

Background: QIX rebranded to CANIX, and the OGIX / National Capital
Internet Exchange entity was amalgamated into CANIX.
