Suggest you save yourself a large amount of future self-loathing and
just use the last octet of the v4 address as the last block of the v6
and thus keep your v6 addresses short and manageable.

Looks like you're still using v4.6.x? That interface has been completely
rewritten in v4.7.0.

You definitely do not want to add a /64 to IXP Manager. You caught that
yourself on the follow up email ;-)

You do not actually need to pre-create addresses - it's a convenience
and con be useful when using a simpler schema like I suggested above.
When you are adding / editing a VLAN interface, you can enter a v6
address in the same form element where the dropdown list is and if it
does not exist, it will be created.
